Medically Necessary Procedures – We accept Medicaid and a limited number of commercial insurance coverages. Please contact our office and/or your insurance carrier to determine if the practice participates with your plan. All insurance is conditioned upon verification of eligibility by the carrier. Co-pays, deductibles, non-covered services, and other patient balances are requested at the time of service. Insurance denials, out of network penalties, and other balances not paid by the Insurance Company are the responsibility of the patient. Self-pay patients or patients who have a large estimated patient due balance who are seeking medically necessary procedures may be required to pay a deposit, however, upon request, our staff will work with patients on a case by case basis in an effort to provide workable monthly payment options when appropriate.

Cosmetic / Elective Procedures – The charge for an initial consult is $100 (non-refundable and payable at the time of the initial appointment) which will be credited towards the cost of a subsequent agreed upon procedure. The patient will be provided with an estimate of charges (including anesthesia if required) and a deposit equal to the full estimated charges will be due in advance of the procedure. A reduction in the cost per procedure may be provided to those patients who have multiple procedures during surgery.
